ATBS PARTNERS PROVIDE THE COVERAGE OFFERED UNDER THIS PLAN:

• Aetna: Aetna will provide health insurance plans under THIS Plan. Aetna is one of the nation’s leading health care companies. Their national network is one of the largest in the country and they are the first major health insurance company to partner with the trucking industry.
• Beta: Beta Health Association is our partner providing dental and vision insurance to truck drivers. Since 1990 Beta has been a leading-edge provider of dental insurance, dental services and dental and vision plans across North America.
• Colonial: Colonial Life & Accident Insurance Company provides drivers supplemental insurance under THIS Plan. Colonial has been providing insurance since 1939 and currently serves over 2.5 million policyholders.

WHY THIS PLAN?

• The underwriting process does not require a physical exam
• Premiums do not automatically rate up for a driver who smokes
• Once you are approved, you cannot be terminated, as long as your premiums are current
• Your premium is guaranteed for 12 months
• There is guaranteed renewal as long as premiums are current
• Coverage is available for spouse and/or children even if you are denied
• All products are month-to-month and you can cancel at any time
• Different plan options let you choose the coverage that is right for you
• The Vision Plan is included FREE with the dental plan
• All products are sold separately, so you get only the insurance you need

Aetna Health Insurance Options:

• Preventive & Hospital Care 1250 – for healthy drivers with no dependents, few or no prescriptions and who want to save on premiums
• Managed Choice Open Access 1500 - (PPO) - for drivers who will have difficulty setting aside money to pay out-of-pocket expenses, have numerous dependents or prescriptions, or prefer the benefit of having a co-pay
• Managed Choice Open Access 3000 or 5000 – (HDHP, HSA Compatible) - for drivers who are only concerned about major out-of-pocket expenses

Beta Dental and Vision Options:

• Care/POS Plan - for drivers who need a comprehensive dental plan—including orthodontists and cosmetic dentistry with unlimited benefits, fee for service, in-network only and no limitations on pre-existing conditions
• TrustMark Value – for drivers who only need basic services. You can see any dentist but this plan does not cover major dental work
• TrustMark Advantage – similar to the Value plan but also covers major work and you can see any dentist

Colonial Accident, Disability and Life Insurance:

• Accident Insurance - supplemental insurance used in addition to Work Comp or OccAcc to help offset medical expenses from a covered accidental injury
• Disability Insurance – individual short-term disability replaces a portion of income to help make ends meet in the event of a serious accident or sickness
• Life Insurance – individual level term life in 10, 20, or 30-year term policy
